Mendieta continued, “Halyna’s loss of life is the results of a large system failure…all of it boils right down to a quite simple query. Why was there a dwell bullet on set? … That’s the place Hannah Gutierrez-Reed failed Halyna. it was her job to examine the gun, examine the bullets, to make sure that Halyna was protected.”
Household pal Steven Metz spoke of how Hutchins’ loss of life devastated her husband, Matthew.
“Matt has been affected horribly by this and he moved away. Principally Matt died when Halyna died… The lack of Halyna has had the ripple impact on our group… her loss of life is a reminder of the fragility of life and left us shaken and unhappy. There may be actually no excuse.”
